Here my Idea.
The protagonist is a High school student with hikikomori tendencies. Sometimes skip the school and so far don't have psychological treatment.
Has a friend male who Doesn't care about anything.jpg and is only a nuisance.
Has a female schoolmate who is Sociopathy/psychopathy manipulative one. In one opportunity exchange words with the protagonist and convinces him that there are some kind of organization behind him just to play with him. Tells him about a forum of conspiracies and there he is put in contact with other people who believe in conspiration.
In this forum there's a unknown guy who tries to use him to help other people, telling X user is an resistance figther.
From there like KS he can contact every character in the story.
So thanks to this we don't have Hospital, neuropsychiatric, prison or anything.
Her Sociopathy/psychopathy schoolmate is one of the heroines. In the forum he can contact to a depressed heroine (maybe with some panic attacks) etcetera.
I can make some research to I know some people in a neuropsychiatric that would be handy explaining what is deal with a psychic problem... but In my mind this VN would be very dark.

Here I repost a little introduction that I came up.
Day after day, I feel useless. I get my breakfast, and go to the school the most depressed place in the world. I skip my class when I can. My parents already gave up on me, the society already gave up on me, those around me, those who are taking the same train.
Today after three days of absence in my school, I decide to return, that made me feel depressed again.
Step by step I’m approaching to the building. All those uniforms, all those absent faces are crossing trough my head and just one think in my mind “loneliness”.
So I’m here standing at the gates again, I’m feel like an outsider here, a gaijin in my own country but I can’t help.
My first class, English. I’m entering in my class. It looks like everyone put their eyes on me, but soon their face are vanished again in their own business. This is my last year here, that’s a relief.
Generally Makoto, my best friend is here, but he doesn’t show up this very morning. Probably has some issues in his home... anyway he generally is more like a nuisance rather than a good company. The English class starts and it feels just empty like usually. At the break our home teacher shows up.
He calls me and another girl, Miki Hayashi. I don’t know here very much. She usually is no very friendly and is the coldest bitch around here. Usually don’t talk with anybody and when she does it ends in a fight or discussion. Has a black short hair, her eyes are disturbing indeed because don’t show any emotions. She is not tall neither short, but when she step in front of you is intimidating.
The teacher is only here for scold us. Apparently like me she had been skipping the school. My parents are aware of this, they just don’t mind. I don’t have bad notes to be worried and sometimes I do my homework after all Makoto bring it me to my house because uses this excuse to get out of his own house, and ruin my day. The scold took the half of our break, and the teacher only leaves while all other students who where in the class saw us.
I’m ready to go when Hayashi stop me holding me by my hand.
“You're not like the others, don’t you?” said Hayashi.
I’m unable to understand her sentence.
“What do you mean?” I said.
“They live their lives without realize of the world that surrounds them.”
Her eyes are slightly different from its usual looks, more like scrutinizing me entirely.
Then she continues “I will keep an eye on you”, make a little smile and she go.
The class continues like always, but I felt the whole time a gaze in my back. When the class is over, I turn to the seat of Hayashi, but she don’t look at me at all. The she left the room and then I return to my home.
The sounds of the train running slowly all the way to my home put thoughts in my head. Something will change on my life soon, is not good being in the spotlight of a person like her.
Then sounds my cellphone. I’ve got an email.
I don’t know the address. The subject is strange “Not point to hide from you”. The address is freespirit95 (at) nani (dot) jp.
The message says “I’m Miki, you know. The truth is always a hidden privilege. Let me enlighten you. Visit the forum. http (doubledotSlashSlash) shinjitsu (dot) bbs (dot) jp . Find yourself your own truth.”
It feels like a prank. I answer in that very moment. “what? Hayashi? How do you get my address? what is the truth?”.
She respond me. “Class rep. Other truths are waiting for you.”
Now I was sure, is a joke.
Finally I’m in my house I call to makoto, his mother just tell me that he is sick, and tell me If I went to the school I must email to makoto our Homework. Evidently she knows that I usually skip my class.
I send our homework and stare to the monitor of my PC. Around my place is a mess. I spend my last three days here until I decided to return to my class.
Then I remembered the forum. I write the direction in the bar address. This site has many threads abut conspiracies. “They know, UFOs are real”...”Mental control vaccines”... but there’s a thread that call my attention “Induced depression & Suicides”.
I went through all the messages. There’s one that has a link to a news site... talk about Scandinavian case of a student’s suicide. The site claims that he commit suicide due he was victim of ciber bulling. A comment below said that there’s an organization with the only objective is turn many people into suicide as possible. There’s no further explanations why or how they can do it.
I subscribe to the forum and make a comment, with a fake name of course, and make a comment about my incredulity of such activity.
In just a second a girl, or at least I think is a girl, the sakura2012 user send me a PM. “Did you play a game last few days? Was A VN? Apparently are using well written VN to make you depressed... and then took control of your mind with subliminal messages”.
I got some other PM, mostly insults for not being a believer of conspiracies.
But that one make through my head. I had played a visual novel in the few past days... and I felt depressed, and I stayed in my house because of it... and I went to the school because I need to get out my thoughts.
Then I got other PM, of an user Sage99... “Probably you don’t believe this, but is not crap... somebody is putting subliminal messages in tv, radio, internet, games, even newspapers. I’m trying to forge a community here, and I have some kind of fighters that can enlighten you if you let them. Apparently someone above want to install pyschological problems into the people. I’m a believer.”
I answer the first PM “there was one. but I didn’t feel manipulated”... but she went offline and I don’t get an answer.
I sent a PM to sage “I’ve got a message of sakura2012 telling me about the games and subliminal messages in VN. But I didn’t bought it”.
He answered me right there.”She went depressed after play a VN, and tried to commit suicide... why do you think there are many hikikomoris here and so many suicides. She is the proof. She saw it and that save her life.”
I just shut down the PC... and I felt bad, then I realized that maybe I was in their hands... then my cellphone... another mail of Hayashi... “did you join? Had a talk with sage?”... I didn’t know how she knew it... that night I had more thoughts than sleep.
